I am announcing my retirement from my private Plastic Surgery practice, The Florida Plastic Surgery Institute effective 09/15/12 at 12:01 AM to begin a position as the first Plastic Surgeon at the new Orlando VAMC Hospital at Lake Nona Medical City. This position will allow me to return to Academic Medicine (UCF College of Medicine) and resume the teaching, medical research and patient care that I practiced before moving to Florida in 2000.
On 09/15/12 at 12:01 AM, George Papanicolaou, MD, FACS, (687 Douglas Avenue, Altamonte Springs, FL 32714; Phone #: 407-478-3151) will be assuming care of my patients. Dr. Papanicolaou is certified by the American Board of Plastic Surgery, is a member of the American Society of Plastic Surgeons (ASPS) and has additional Fellowship Training in both Hand Surgery and Craniofacial Surgery. I am fortunate to have left my patients in his capable hands. Of course, you may seek medical care from another Board Certified Plastic Surgeon/ASPS Member. If you choose to do so, I recommend looking for a new Plastic Surgeon as soon as possible. The names of other Board Certified Plastic Surgeons who are members of the ASPS can be found by visiting www.plasticsurgery.org .
Your medical records are confidential, and a copy can be transferred to another Plastic Surgeon or released to you or another person you designate only with your written permission. If you plan to receive follow-up from Dr. Papanicolaou, please complete, sign and return the enclosed authorization form to: Florida Plastic Surgery Institute, 5036 Dr. Phillips Blvd., Suite 119, Orlando, FL 32819 to release a copy of your medical record to Dr. Papanicolaou. If you choose to seek follow-up care with a different Plastic Surgeon, please complete and sign the enclosed authorization form and return it to the above address so we may send a copy of your medical record to your new Plastic Surgeon. Until then, your records will remain on file in a secure locked file cabinet compliant with HIPPA regulations.
